COVID-19 Information

On behalf of my staff and myself, I hope you are faring well during these challenging times. We have been very concerned about all of you. Our entire staff has been hard at work setting up protocols that conform to CDC guidelines. We are now seeing patients on a limited basis, and our top priority is ensuring your safety and the safety of our team.
To that end, we are instituting the following:

  • Please brush your teeth at home. We will no longer offer the ability to brush your teeth when you get to the office.
  • Please plan to arrive at least 15 minutes early for your scheduled appointment time. We need to allow for additional time between patients to completely disinfect everything after patient visits, and we also need to minimize the number of patients in the office at the same time. If you are late due to traffic, etc., we may need to turn you away.
  • Please use the bathroom at home prior to arriving at my office. If you absolutely need to, we will allow you to use our patient bathroom, but since we will need to completely disinfect it after use, this will take additional time, and take staff away from other assigned duties, and impact our ability to stay on time.
  • When you get to the parking lot, you will call our office phone number to let us know you are here, and give us your cell number. Please remain in your car until we call you to tell you it is time to come in.
  • Only one parent will be allowed to accompany their minor child to their appointment. Please try to arrange it so additional family members don’t need to come in. Children will not be allowed to run around the office and potentially contaminate areas. The video game area in the waiting room and play area in the treatment room can no longer be used.
  • We have set aside all of the waiting room chairs because they are not easily disinfected. People accompanying the patient will need to stand in designated areas.
  • Everyone who enters the office should be wearing a mask that covers your nose and mouth. Gloves do not provide any protection from a patient’s perspective, and when you enter the office, if you are wearing gloves, we will require you to remove them and use hand sanitizer that we will provide (everyone needs to use hand sanitizer when entering and leaving the office).
  • Everyone’s temperature will be taken with a non-contact infrared forehead thermometer upon entering the office.
  • We will ask you questions about your current condition, and possible recent exposure events, and require you to sign a consent form at each visit (the questionnaire link is provided below. You don't need to fill this out at home - we will be providing the form at each visit). We may, at our discretion, ask you to reschedule your visit if we feel that there is a risk to us or other patients. We will provide a disinfected pen to sign the form. There will be a container on the floor to place the pen after you are done using it.
  • Please call the office if you are unable to keep your appointment (don’t just no-show) or if you are going to be late.
  • With the additional cost of PPE being incurred by my practice, any procedure that generates aerosols will result in a $10 PPE fee.

Please click the link below to see the Questionnaire that we are currently using. You will be asked to sign and date this at each visit.

We appreciate your understanding and cooperation, and look forward to seeing you soon.
-Dr. Silverstein
